Health Coach Care Coordinator

Fully virtual | Full-time | $20–$22/hr

As a Health Coach Care Coordinator, you'll build meaningful relationships with patients navigating chronic conditions — helping them set goals, make sustainable lifestyle changes, and overcome the obstacles standing in their way. All of your work happens virtually through our platform so you can provide care without being in a hospital.

Key Responsibilities - Health Coach Care Coordinator

What you'll do:

• Be a trusted guide for patients managing chronic conditions, supporting them through education, encouragement, and practical problem-solving
• Use motivational interviewing to meet patients where they are and help them move forward on their own terms
• Connect with patients via phone, video, and messaging — building genuine rapport over time
• Collaborate with care teams to ensure every patient gets coordinated, high-quality support
• Direct patients to the right resources and tailor your approach to each person's unique history and goals
• Chart and document patient interventions, provider interactions, and general clinical notes

Education and Experience - Health Coach Care Coordinator

What we're looking for

Required:

• 2+ years working directly with patients as a Medical Assistant or other healthcare professional
• High school diploma or GED
• Comfort with technology and virtual platforms
• Health coaching certification, or the drive to earn one (fully covered by us)

Preferred:

• CMA certification from a nationally recognized organization (or prior cert with willingness to renew)
• Associate degree or higher in medical assisting
• Prior health coaching or consultation experience
