Key Responsibilities - Care Coordinator

What You'll Do:

As a Remote Care Coordinator, you'll serve as a vital link between patients, providers, and the therapies that can change their lives. Responsibilities include:

• Handle inbound and outbound calls from patients and referral sources — aiming for one-call resolution
• Steward patient accounts from initial contact through final approval or denial with urgency and care
• Create and complete accurate referrals and applications; stay current on policy and procedural changes
• Investigate and resolve patient and physician inquiries and concerns in a timely manner
• Enter detailed information into proprietary software systems while conversing via telephone
• Place outbound calls for patient follow-ups and confirmations
• Report adverse events within required timeframes
• Educate patients on their options and guide them through processes in clear, plain language
• Prioritize multiple concurrent assignments while maintaining quality and an empathetic patient experience

Education and Experience - Care Coordinator

What You'll Bring:

• 1–3 years of experience in a related field
• Pharmacy experience required
• Previous customer service or call center experience preferred
• High school diploma, GED, or equivalent work experience
• Knowledge of Medicare (A/B/C/D), Medicaid, and commercial payer policies preferred
• Familiarity with Medical, Supplemental, and pharmacy insurance benefit practices preferred
• Understanding of Diagnostic Medical Expense and Medicare Administrative Contractor practices preferred
• Excellent written and verbal communication skills; ability to connect with patients, caregivers, and providers
• Strong people skills that demonstrate flexibility, persistence, creativity, empathy, and trust
• Proficiency in MS Office and data entry; strong computer literacy
