What You'll Do

• Schedule and treat home health patient caseloads with compassion and precision.
• Deliver personalized care plans that promote mobility and independence.
• Document visits accurately using Matrix/Brightree (training provided).
• Collaborate with a multidisciplinary team to ensure continuity of care.

Required Skills and Knowledge- Interim Home Health Physical Therapist Assistant (PTA )

What You Bring

• Active Michigan PTA License (required).
• Home health experience preferred, but training provided.
• Strong time management, communication, and adaptability skills.
