Key Responsibilities - Case Manager

What You'll Do:

As a Remote Case Manager, you’ll guide patients and healthcare providers through important processes to ensure smooth, timely outcomes. Responsibilities include:

  • Handle inbound and outbound calls from patients, providers, and specialty pharmacies — aiming for one-call resolution
  • Manage the full care journey from benefit investigation through medication delivery with urgency and care
  • Conduct benefit verifications and coordinate with physicians, specialty pharmacies, and insurers
  • Assist with prior authorizations, insurance requirements, and appeals — keeping patients informed throughout
  • Explain insurance coverage, out-of-pocket costs, and the appeals process to patients in plain language
  • Stay current on Medicare, Medicaid, Managed Care, and commercial plan changes to proactively support patients
  • Process enrollments via fax, phone, and electronically; review forms for accuracy and completeness

Education and Experience - Case Manager

What you Bring:

  • 2–4 years in patient-facing or high-touch customer service
  • Previous Hub or Patient Support Service experience preferred
  • High school diploma or equivalent
  • Knowledge of Medicare (A/B/C/D), Medicaid, and commercial payer policies preferred
  • Familiarity with pharmaceutical therapies, disease states, and medication adherence challenges
  • Strong written and verbal communication skills; ability to connect with patients, caregivers, and providers
  • Proficiency in MS Office and data entry; strong computer literacy
  • Strong people skills that demonstrate flexibility, persistence, creativity, empathy, and trust.
